Food Assistance Registration

Register your household for food distribution and community resources in Phoenix.

Thank you for your interest in the Amar’e Stoudemire Foundation Community Resource Program. Our mission is to strengthen families by providing access to food assistance, education, financial literacy, workforce development, wellness resources, youth programming, and other community services.

Completing this form helps us better understand your household’s needs and connect you with available resources. This application does not guarantee food availability. Food assistance is based on available inventory. Registered households will receive priority notification for upcoming distributions.

Our building is located at 5830 N. 19th Ave, Building A, in Phoenix, 85015. Distribution happens on Sundays from 1 to 4 PM with St. Mary’s Food Bank.

Please note: we are not taking any new delivery requests at this time, and Wednesday distribution is on pause. All Wednesday requests are being served through Sunday distribution.
